A new Taiwanese cohort study asks whether physical fitness should be treated as more than a lifestyle outcome in older age. Published in JAMA Network Open on August 10, it linked standardized fitness measurements from 13,423 community-dwelling adults aged 65 or older with national health and death records over a median follow-up of seven years. The objective assessment covered several domains rather than relying on a single exercise measure. Participants completed tests of cardiorespiratory fitness, arm and lower-body strength, flexibility, balance and agility. The researchers then examined how performance related to all-cause mortality after adjusting for factors including age, sex, medical history, body mass index, socioeconomic measures and self-reported physical activity. Higher overall fitness was associated with lower all-cause mortality. Among the individual tests, balance and agility, lower-body strength and cardiorespiratory fitness showed particularly strong associations with lower risk. This does not turn a chair-stand score into a prediction of how long one person will live. The research is an observational study, so it identifies association rather than proving that improving a specific test directly prevents death.
